Clinic Appointment

You may call our appointment center at (901) 545-6969 between 8 am and 4:30 pm, Monday through Friday, to make, cancel or reschedule an appointment.

In order for us to better assist you, please have the following information available before you call:

• Patient's name and social security number
• Patient's address and telephone number
• Patient's primary care physician or referring physician for all specialty services
• Name of the patient's insurance carrier and phone number
• Patient's insurance identification number
• Patient's authorization or referral number, if required

If you have to cancel your appointment, please call us at least 24 hours ahead of time so that we may make your appointment available for another patient.

Patients more than 30 minutes late may be rescheduled.
