Make an Appointment

To make an appointment for outpatient imaging services or for an office visit in one of our speciality physician clinics, please call the appointment center Monday – Friday from 8 a.m. to 4:30 p.m.

In order for us to better assist you, please have the following information available before you call:

Patient's name and social security number
Patient's address and telephone number
Patient's primary care physician or referring physician for all specialty services
Name of the patient's insurance carrier and phone number
Patient's insurance identification number
Patient's authorization or referral number, if required

If you have to cancel your appointment, please call us at least 24 hours before time so that we may make your appointment available for another patient. Patients more than 30 minutes late may be rescheduled.
