Insurance, Billing and Payments


We will happily file insurance claims. Payment for patient care services is always the patient's responsibility. Here are a few payment details to keep in mind:

  • If the patient’s insurance requires a co-payment and/or deductible, please be prepared to pay it at the time of service.
  • Self-pay patients must pay a deposit at the time of service and will be billed for the balance. The appointment scheduling staff can provide the amount of the deposit required.
  • There may be a separate bill for physician services.
  • Payments may be made with a personal check, Visa or MasterCard.

Lab


At the Outpatient Center, we feature a lab for the convenience of our patients. If your physician asks you to have lab work completed, be sure to:

  • Have it completed at least one week prior to your next scheduled appointment, or two weeks if you are a patient of the Adult Special Care Center. The lab work will be necessary to assist the doctor with your medical care.
  • When arriving to have your lab work completed, bring the lab order your doctor gave you to the pre-admission testing desk located on the second floor of the Outpatient Center.

Registration Process


Insurance information will be obtained at the time an appointment is scheduled and then verified during the pre-registration process. The patient must bring two forms of identification (preferably a driver's license and government issued social security card) as well as insurance cards to each clinic visit. The patient will be contacted two to three days prior to the scheduled appointment to review insurance and personal information and will also receive a reminder telephone call within three days of the scheduled appointment.
